Hello doctor,

I am a 55-year-old female who is currently overweight, though I have successfully lost 60 pounds and aim to lose an additional 100 pounds. I recently received my CBC results, and I noticed that my immature granulocyte levels are higher than the normal range. Specifically, my immature granulocyte count is 0.6, exceeding the upper limit of 0.3 and my immature granulocyte absolute count is 0.04, surpassing the upper limit of 0.03. All my other white blood cell counts are within the normal ranges. My previous blood tests did not indicate any presence of immature granulocytes. In terms of medication, I am taking Lisinopril, NAC, Tudca, and Berberine. While I am not experiencing any symptoms, I am currently dealing with plantar fasciitis. Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

I have gone through the reports you have attached (attachments removed to protect the patient's identity). I want to reassure you that there is no immediate cause for alarm. The elevated levels of immature granulocytes could potentially be attributed to the inflammation and fasciitis you are currently experiencing. Have you undergone an abdominal ultrasound to assess the condition of your spleen and liver? It would be helpful to know if there is any history of blood cancer or blood disorders in your family.

Your response to these inquiries will allow me to offer you more precise guidance.

Thank you.

Hi doctor,

I appreciate your response.

My father was diagnosed with a form of blood cancer before he passed away due to uncontrolled diabetes. He used to self-administer stomach injections to manage it. Unfortunately, I am not aware of the specific type of blood cancer he had. He was 56 years old at the time of his passing. On the other hand, my mother does not have any history of blood disorders.

As for my medical history, I have not undergone any ultrasounds to assess the condition of my spleen or liver. But, I have been informed that I have gallstones, and I believe I recently passed one about a month ago. My liver function test results have remained consistent over time. If it would be beneficial, I am willing to share those specific test results with you.

Thank you for your attention and guidance.

Hello,

Welcome back to icliniq.com.

Kindly send me your liver function test and ultrasound reports. I will be happy to help you further.

Thank you.

Hello doctor,

Thank you for your reply.

I do not possess any recent ultrasound results as it has been more than a decade since my last ultrasound. I will provide you with my recent liver bloodwork results. Notably, my ALP level is slightly elevated and this specific value has remained relatively consistent over the course of approximately five years.

Thank you.

Hello,

Welcome back to icliniq.com.

I have reviewed your results (attachments removed to protect the patient's identity) and appreciate the information. The elevated ALP (alkaline phosphatase) level can largely be due to your obesity, which seems to be a contributing factor. Given your weight being on the higher side, it is important to understand if obesity has been a long-standing concern for you.

It would be helpful to know if you engage in any habits such as smoking or alcohol consumption.

Obesity can potentially lead to anomalies in blood work, including the presence of premature granulocytes.

Thank you.

Hello doctor,

Thank you for your reply.

I do not smoke, and my alcohol consumption is infrequent. I have struggled with obesity for the past 30 years. But, in the last year, I have actively embraced exercise and managed to shed 60 pounds thus far. My A1C level stands at 5.1, marking a decrease from its previous value of 5.6. With this progress, I am hopeful that my weight will continue to decrease in a positive trajectory.

Thank you.

Hello,

Welcome back to icliniq.com.

It is noteworthy that you have abstained from smoking and have limited alcohol consumption. Your dedicated efforts towards weight loss are good. I encourage you to persevere in your journey and reach your desired weight goal. Regarding your concerns about premature granulocytes, there is no immediate cause for worry.

Would it be feasible for you to arrange for a recent ultrasound of your abdomen? This could provide valuable insights into the condition of your liver and spleen.

Thank you.
